NEW ADDRESS FOR ALL MEDICAL CLAIMS

 All claims should be submitted to this address. As much as possible encourage your hostfamilies to have doctor's offices and ERs bill Global directly .  Otherwise, here is an updated version of the medical claim form if you need to seek reimbursement.

I am listing both the old phone number and the new phone number. Although the old number will be answered, please start asking folks to use the new phone number which is specifically for Global Benefits.

Please indicate to the person who answers the phone  how you are affiliated with AFS (eg, hostmom, volunteer).
